What is an assessment for care and support?

Sometimes our parents/carers need support to care for you. We will work alongside you and your family to make sure you are kept safe and well cared for.

We will want to know what you/ your family think is important and what you would like to change (if anything). We will also make sure that your education and health needs are well cared for.

What is a Care and Support plan?

If suitable, we will help support you and your family with a Care and Support plan that says how you will receive support to make sure you are safe, healthy and happy.

Who might be part of that plan?

You/ your family should be involved in developing the plan and what matters to you should be a big part of it. 

Others that are likely to be part of developing the plan include teachers and health visitors involved in your life. The kind of support that might be on the plan include supporting you with routines, support with school and the support you may need.
